If you would like to pick up a copy of your 2015 contribution statement, please call the parish office (587-44372). Your statement will be available for pick-up after 2 PM on the Friday following your call.

If you would prefer to have your statement mailed to you, please put a business-sized self-addressed stamped envelope (SASE) in the collection basket or mail one to or drop one off at the parish office (beneath OLS church). Statements will be mailed only to those who have provided an SASE.
