If you haven't already done so, please submit, prior to June 30, all requests for your group's use of facilities at Our Lady of Sorrows and/or St. Anthony campus from July 2016 through June 2017. Information and forms, including an annotated calendar facility reservation form, are available on the
Reserving Parish Facilities page under News, Etc.
Requests may be sumitted by sending a copy of the completed form to the parish office (OLS-SA, 3816 E. State St. Ext., Hamilton, NJ 08619) or by emailing the information to
[email protected].
When emailing the information, please provide the name of your organization or ministry, and the phone number and email address for your group's primary and secondary contact persons. Indicate the type of each event or function (e.g., meeting, dinner) and the dates and times (beginning and ending) involved. If additional time is needed to set up for the event and/or clean up after it, please specify when you need to begin setting up and when the facility will be cleaned up. Provide an estimate of the number of attendees, and indicate your room preference, if any.
